CHC provides skilled professional and paraprofessional home health care services throughout Nassau, Suffolk, and Queens Counties. Specialty services include skilled nursing, physical therapy, occupational therapy, speech language pathology, nutritional counseling, medical social work, and home health aides. We have program specialties with highly trained staff to include pastoral care, behavioral health, infusion therapy, wound management, palliative care, and telehealth.

Having held managerial roles within the community services industry for over two decades, Kylie is an experienced member of our executive team in the role of Executive Manager Home and Community Care. Managing multidisciplinary teams of staff and volunteers, Kylie is responsible for the development and implementation of new programs, risk management, improvement and reporting to Government and non-government departments.
